INFORMATION FOR CARRIERS

PLEASE TAKE NOTE OF THE FOLLOWING:

At KL Parker Logistics, we highly value our partnership with each of our carrier associates and we fully comprehend the importance of timely payment. Our policy at KL Parker Longistics is to ensure that all carriers are paid within 30 days upon receipt of their invoice and Proof of Delivery. Before any load is tendered, we require the following information from each carrier:

• Valid operating authority of the carrier

• Dot number and Safety Rating

• Certificate of Insurance, with the amount limits, deductibles, and expiration dates

• Duly signed W-9 form

• Signed Broker/Carrier Contract.
